Information:	Целью разработки данного комплекса является создание и внедрение системы расширения практики грудного вскармливания на всех этапах оказания медицинской помощи женщинам и младенцам. Опираясь на десять принципов успешного грудного вскармливания, которые изложены в Совместной декларации ВОЗ/ЮНИСЕФ "Охрана, поощрение и поддержка практики грудного вскармливания: особая роль родовспомогательных служб" (1989) для достижения поставленной цели был определен круг задач, представленных на схеме 2. Комплекс может быть реализован в полном объеме или частично, на уровне всего региона или на уровне конкретного ЛПУ. Эффективность решения каждой задачи может быть измерена специфическим индикатором, что позволит вам отслеживать успешность решения выбранной задачи. В свою очередь решение каждой задачи может быть достигнуто путем изменения (или создания) отдельных позиций, собственно и определяющих решение поставленной задачи. Общий список индикаторов, по которым проект «Улучшение помощи мамам и малышам» оценивает эффективность достижения цели, представлен в 4-й главе, совместно с подробными инструкциями по их сбору.

Information:	В данном документе объединен практический опыт нескольких регионов, участвующих в проекте (Кострома, Тамбов, Ярославль) по вопросам расширения практики грудного вскармливания для гармоничного развития и улучшения здоровья детей грудного и раннего возраста, а также опыт внедрения Инициативы ВОЗ/ЮНИСЕФ «Больница, доброжелательная к ребенку» в России Научно-практического центра по пропаганде, поддержке и поощрению грудного вскармливания Минздравсоцразвития РФ. Описание актуальности охраны, поддержки и поощрения грудного вскармливания и формирования культуры грудного вскармливания в обществе представлено в 1-й главе. 2-я глава посвящена вопросам физиологии лактации, технике кормления грудью, принципам успешного кормления грудью и сохранению лактации в сложных ситуациях, важности исключительно грудного вскармливания в первые 6 месяцев жизни младенца. В 3-й главе перечисляются конкретные организационные действия, которые необходимы для успешного внедрения рекомендованных технологий по улучшению практики грудного вскармливания в ЛПУ родовспоможения и детства с учетом современных рекомендаций российских и зарубежных ученых, материалов Всемирной организации здравоохранения, Постановлений Правительства Российской Федерации, приказов и инструкций Минздравосоцразвития России. Индикаторы оценки результатов, мониторинг и оценка внедрения предложенных технологий приведены в 5-й главе. Необходимые ресурсы для внедрения рекомендованных действий описанны в 6-й главе и конкретный план внедрения ПГР по срокам - в 7-й главе. В 8-й главе кратко описаны барьеры в реализации поставленных задач, которые выявили пилотные команды. Полезные материалы, инструменты и примеры документов находятся на диске приложений, включая презентации пилотных ЛПУ с демонстрацией основных этапов работы по проекту и достигнутых результатов. Ценность предлагаемых организационно-методических решений в том, что они были разработаны, апробированы, скорректированы на основе практики и внедрены вашими же коллегами параллельно в нескольких районах, областях и ЛПУ разного уровня в разных регионах. Их применение продемонстрировало высокую эффективность, подтверждающуюся тщательными измерениями. Целью разработки данного пакета готовых решений является создание и внедрение системы расширения практики грудного вскармливания на всех этапах оказания медицинской помощи женщинам и младенцам.

Information:	WHO and UNICEF jointly developed the Global Strategy for Infant and Young Child Feeding to revitalize world attention to the impact that feeding practices have on the nutritional status, growth and development, health, and thus the very survival of infants and young children. The Global Strategy is based on the evidence of nutrition’s significance in the early months and years of life, and of the crucial role that appropriate feeding practices play in achieving optimal health outcomes. The Strategy is intended as a guide for action; it identifies interventions with a proven positive impact, it emphasizes providing mothers and families the support they need to carry out their crucial roles, and it explicitly defines the obligations and responsibilities in this regard of governments, international organizations and other concerned parties.

25. "Breast-feeding counseling" 40-hour course by WHO/UNICEF

Information:	The main purpose of the course "Breast-feeding counseling" is to narrow gap in knowledge on the importance of breast-feeding among medical workers. Materials are composed in such a way that even trainers with little of experience in this field can conduct a training effectively and in accordance with the modern standards. The course should take 40 hours and can be done in a series of day after day training during the week or any other way. The course is subdivided into 33 lessons 30 and 20 min each with usage of different teaching methods, what includes lectures, demonstrations, clinical practice, and small group discussions combined with readings, play, etc. The shortest lessons consist of four 2-hour practical experience at the clinics. The participants gradually improve their ability to execute concealing in classrooms and than apply their knowledge during their practical classes with mothers and children in hospital units. Our project will consider the implementation of this course if requested.

Information:	The purpose of this document is to combine knowledge and expertise on main approaches how to arrange the work on breast-feeding support and promotion and to inform the executives in healthcare and working doctors in this field about these approaches. The guideline for doctors decribes 10 principles (steps) of successful breast-feeding, which can be applicable to different medical and preventive facilities of obstetrics and childhood. It also includes the desciption of different approaches used in Russia and authors' own experience in the field of breast-feeding promotion and support. It contains recommendations how to prevent hypogalactia and recommendations on what food the women who breastfeed should eat. It also provides a good reference list to the works of different international and Russian specialists working in this field.
